The Private School Affidavit (formerly the "R4" form) is the form to register a private school you have established for your children, with the California Department of Education at www.cde.ca.gov/sp/ps/rq/affidavit.asp. Local independent study programs (ISP's) and homeschool charters are options you may wish to investigate.
